These guides are the first stage of our Brisbane suburb-page expansion. They help families find local support options near real parks, shops, transport and community places, but they are not a boundary around where HSN can help.
Northside support around village precincts, major shopping, parks, bikeways, hospitals and bus and train connections.
Eastside support around shopping centres, parks, libraries, bayside walks and train and bus access.
Westside support around the river, shopping, university access, trains, buses and ferry trips.
Southside and south-west support around Garden City, Sunnybank, Holland Park, Inala, Acacia Ridge, food precincts, parks and public transport links.
The original inner-south suburb guides, close to the river, hospitals, parks, cafes and the CBD.

We support people across Brisbane and beyond. The suburb pages listed here are local guides, not boundaries. If your suburb is not listed, send an enquiry and we will talk through fit, travel and availability.
No. Horizons Support Network is a non-registered NDIS provider, supporting self-managed and plan-managed NDIS participants. We do not support NDIA-managed funding.
We serve all of Brisbane, including north, east, west, south and inner-south suburbs. Support depends on the right worker fit, timing and travel, not a fixed postcode boundary.
We provide three ongoing support-worker services: Daily Personal Activities, Independent Living Skills, and Social & Community Participation. We also provide Short Term Respite as separate, planned support. We do not provide Support Coordination, Supported Independent Living or clinical services.
