Chris' support and encouragement of Xavier has been instrumental in him obtaining work as a volunteer with RSPCA. Xavier's pride at having a 'job' is heartwarming to see. Xavier is always keen to head out the door with Chris, he comes home with tall tales and a huge smile, looking forward to their next adventure. As a family we feel extremely fortunate to have a man like Chris in Xavier's life, he is a wonderful role model, support worker and treats all with respect.

We help participants build confidence, friendships and life experiences through meaningful community access across Brisbane, from a first quiet outing to joining a local club. We move at a pace that suits each person and shape every plan around the things they actually want to do.

How this is funded in an NDIS plan
This support may be funded under Assistance with Social and Community Participation in Core Supports when it is reasonable and necessary and in the participant's plan. It is not clinical support and it is not Supported Independent Living (SIL). Horizons is a non-registered provider, so this is for self-managed and plan-managed participants only, when it fits the plan and the service agreement. Read what NDIS core supports cover if you are sorting the budget, or Daily Personal Activities if the need is home and personal care rather than getting out.

Social & community participation, answered
What is NDIS social and community participation?
NDIS social and community participation supports help participants engage in activities outside the home, build friendships, develop skills and become more involved in their local community.
What activities can Horizons support with?
Our team can assist with social outings, community events, hobbies, fitness activities, group programs and everyday experiences that help participants build confidence and connection.
Do you provide support across Brisbane?
Yes. Horizons Support Network provides NDIS social and community participation support across Brisbane and surrounding areas.
Who can use Horizons for social and community participation?
Horizons supports self-managed and plan-managed participants only; we do not support NDIA-managed participants. The support must fit the participant's current plan and service agreement. A funded Support Coordinator can help connect and implement supports, while a plan manager can help with budget, invoice and payment questions.
For support coordinators
HORIZONS Support Network is a non-registered NDIS provider in Brisbane supporting self-managed and plan-managed participants only; HSN does not provide support coordination.
The participant chooses their providers, and there are no referral rewards.
With the participant's consent, send your organisation, role, a contact method, plan management type, the person's diagnosis or disability, and a brief outline of support needs; please leave out NDIS numbers and surnames.
