What does self-managed mean?
Self-managed means the participant, nominee or representative manages payment and records. This usually gives more provider choice, including the option to use non-registered providers when the support fits the plan.
The practical answer: Horizons can support self-managed and plan-managed NDIS participants. We cannot support NDIA-managed participants because HSN is a non-registered NDIS provider.

NDIS participants can have funding managed in different ways. The plan-management type affects how providers are paid, what admin is involved and whether a participant can use a non-registered provider like HSN.
Self-managed means the participant, nominee or representative manages payment and records. This usually gives more provider choice, including the option to use non-registered providers when the support fits the plan.
Plan-managed means a plan manager handles invoices and payment admin. Participants still usually have choice to use non-registered providers where the support is appropriate and the plan manager can process it.
NDIA-managed means payments are managed through NDIA systems. This usually requires registered providers, so HSN cannot support participants whose relevant funding is NDIA-managed.
Horizons can support self-managed and plan-managed participants across Brisbane. Horizons cannot support NDIA-managed participants.
This is because HSN is a non-registered NDIS provider. We keep that clear because it helps families work out fit quickly, without confusion or pressure.
If only part of your plan is self-managed or plan-managed, the answer may depend on which budget would fund the support. Your plan manager, nominee or NDIA contact can help confirm that.
Check how your relevant funding is managed, what support category the support sits under, and whether the provider offers the kind of support you actually need. You do not need to have every answer before contacting HSN.
A useful first message is: "My plan is self-managed" or "My plan is plan-managed", followed by where you live in Brisbane and what kind of support you are looking for.

Horizons provides three support-worker services only: Daily Personal Activities, Independent Living Skills, and Social & Community Participation. We deliver those supports across Brisbane for self-managed and plan-managed participants.
HSN cannot provide supports through NDIA-managed funding. You can ask your NDIA contact or plan representative about your plan-management options, but HSN cannot tell you to change your plan or promise that a change will be approved.
